Any one of the following may be used to confirm identity:

• United States Passport, or 
• School ID card or records, or 
• United State Military ID, or 
• Birth Record, or 
• The Department will accept other reasonable proof of identity - call 1-877-456-1233 with questions.

• If you are self-employed, your most recent personal income tax returns or a completed Self-employment Verification form.
• A signed statement from your employer
• Proof of child support, if paid from another state, such as a printout from the state's child support agency
• Proof of veterans benefits, out of state unemployment, retirement income, workers compensation, or any other income that is not earned

• Printouts from your bank that include your name, account number, and balance
• Printouts or statements that include current value of stocks, bonds, certificates of deposit, and trust accounts

• Prescription receipts
• Statement from a hospital, doctor, or other medical provider

• A residency document issued by the Immigration and Naturalization Service (INS) or the Department of Homeland Security

• Written statement from the person receiving the payments
• Court documents
• Statement from the state child support agency, if not paid through Idaho

• Statement from your child care provider
