If your EMS personnel license expires September 30, 2014 please submit your renewal application as soon as possible. You can check the status of your application: Received Applications

Initial License

Initial Provider Application


Thank you for your interest in becoming a licensed Emergency Medical Services (EMS) Provider.  Licensure by the Idaho EMS Bureau is required for you to perform patient care in Idaho.  The following steps will help you with the licensing process.

Step 1: Successfully complete your EMS Course.

Step 2: Pass the National Registry of EMT’s (NREMT) computer based written examination and the practical examination.  Successful completion of these examinations is required to apply for an Idaho EMS license.

All candidates must register online with NREMT: NREMT Website Instructions for candidate registration for the computer based written examination at a Pearson VUE Test Center: Student Instructions for NREMT EdNet Registration. Pearson VUE Centers are located in Boise, Coeur d’Alene, Idaho Falls, Lewiston, Pocatello, Twin Falls, and Spokane, Washington. Pearson VUE Centers are secure testing locations and might be a little intimidating. This video will help explain-What to expect in a Pearson VUE Authorized Test Center.

The practical examination is a separate event from the written. Instructions for candidate registration: Practical Exam Brochure. All practical examinations require pre-registration with a Candidate Practical Examination Application. This must be done in advance to guarantee a spot in the exam. Some host sites charge a fee for the practical examination. It is the candidates’ responsibility to arrange for fee payment with the host site. The practical exam schedule is posted on the Education webpage and updated weekly.


Step 3
: Submit to and pass a Criminal History Check by the Department of Health and Welfare.  Use Employer ID# 1350. You can access their website at: https://chu.dhw.idaho.gov

Step 4
: Secure affiliation with an Idaho licensed EMS agency.


Step 5: Complete and submit an Initial Provider Application; indicate the level of licensure you are seeking.

    •  Include a copy of your photo ID.
         -Acceptable forms of ID are: driver’s license, state identification card and military identification card. 

    •  Answer the two questions about other state licensure/certification. 
         -If you answer Yes to the first question, send an Idaho EMS License Verification Request form to any/all states where you have held or applied for an EMS license. The form will be returned to the Idaho Bureau of EMS & Preparedness office by the verifying state.

    •  Advanced EMT and Paramedic applicants: fill out Direct Bill information or include fee payment by cash, check, or money order made out to Idaho EMS.
        

License Renewal

Please do NOT send in any extra paperwork other than what is requested. All continuing education is self-declared.  Please do not include certificates or rosters, etc.  It is advisable for providers to have their certificates and proof of completions in case there are any questions or if the Bureau requests an audit, but they are not required to be submitted for license renewal. The additional paperwork really slows down the processing time of the applications.

Renewal Education

Go to TRAIN Idaho (use Internet Explorer 7 or higher or Google Chrome). If you had an account at Idahoprepares.com use your email address and password from your Idahoprepares.com account to access TRAIN Idaho. If you did not have an account with Idahoprepares.com or you are unable to log in, set up an account with TRAIN Idaho.
Go to TRAIN Idaho (use Internet Explorer 7 or higher or Google Chrome). If you had an account at Idahoprepares.com use your email address and password from your Idahoprepares.com account to access TRAIN Idaho. If you did not have an account with Idahoprepares.com or you are unable to log in, set up an account with TRAIN Idaho.
Once you have completed the extrication awareness training, your completion data can be found in your transcript under "My Learning" or you can print a certificate under "My Certificates".
