The Idaho Department of Health and Welfare licenses State Funded Treatment Providers to offer services through the Substance Use Disorder Program. 
 
How do I become an approved State Funded Treatment and/or Recovery Support Services (RSS) Provider?
 
Mail an Application for Facility Approval or fill out an Electronic Application for Facility Approval online and send it along with an electronic copy of your policies and procedures according to IDAPA 16.07.20

How do I become Qualified Substance Use Disorder Professional?

Clinicians who provide direct client substance use disorders (SUD) treatment services by a Department of Health and Welfare approved substance use disorders treatment agency, must be approved as a Qualified Substance Use Disorders Professional  (QSUDP) or a Qualified Substance Use Disorders Professional Trainee (QSUDPT) in accordance with IDAPA 16.07.20 subsections 218 or 223

For QSUDP approval:  Mail or fax your current State of Idaho Occupational License or Certification as listed in IDAPA, resume and proof of 1040 hours Substance Use Disorder hours                    

For QSUDPT approval: Mail or fax your current State of Idaho Occupational License or Certification as listed in IDAPA, resume and intensive supervision plan

Yes, all staff members, volunteers, and interns must have a TB test upon employment and annually thereafter. In addition, each agency must establish policies and procedures to ensure the proper documentation of this testing. The purpose, of course, is to reduce the spread of TB by identifying and treating those with active (infectious) TB. These measures are particularly important when treating individuals for substance abuse/addiction. Due to suppressed immunity, research indicates that those with a history of substance abuse are at significantly increased risk to 1.) become infected, 2.) exhibit more severe symptoms (cough, weakness/fatigue, chills, fever), and 3.) respond poorly to treatments. For this reason, staff members that have active TB must be restricted from employment until it is determined by laboratory evaluation that the TB is non-infectious.
