Projects for Assistance in Transition from Homelessness (PATH) Subgrant

There is a funding opportunity available for Providers.  Providers who wish to apply for the Projects for Assistance in Transition from Homelessness (PATH) Subgrant should review the application and announcement.  The funding opportunity is for $190,000 to help support service delivery to individuals who: 
1.    have a Serious Mental Illness (SMI) or co-occurring SMI and Substance Use Disorder (SUD); and
2.    are experiencing homelessness or at imminent risk of homelessness.
